We are delighted to meet you! In order to allow our team of dentists to serve you best and to know your dental situation, it is necessary to fill out a form with the relevant information about your state of health. As a new patient, you can complete the confidential medical questionnaire by clicking on the link below. If you cannot complete it, we ask you to arrive 15 minutes in advance so that you can complete it on site. If you are taking medication, we suggest that you bring this list with you so that we can review it.

The course of the first visit :

Dental exam

The dentist will perform a complete dental exam and ask you questions about your medical and dental history. The dentist will examine your teeth, gums, occlusion and jaw. If he/she deems it necessary, dental x-rays and screening for oral cancer may be performed.

Treatment plan

The dentist following the examination will define a dental treatment plan adapted and personalized according to your needs.  He will also take the time to discuss with you to better understand your needs and expectations regarding the services and treatments that may be offered.

Insurance and Financing

At Centre dentaire Cousineau, we want everyone to have access to dental care. That’s why we offer different types of financing as well as billing for your insurance.

Insurance claims

Our team listens to you and knows the different insurance plans, so we can provide you with direct billing and explain how it works if needed.

Means of payment

The cost of the treatment in its entirety must be paid at the time of the appointment. We accept payments in cash, debit and credit card.

Financing available

At Centre dentaire Cousineau, we offer several financing solutions, including financing with Dentalcard. In both cases, the process is confidential and personal and is carried out directly with the company in question. If you have any questions about the different financing options, we are available. We will be pleased to explain the different options.

Governmental Programs

Our team of dentists is trained to provide care to patients covered by various programs.

Medically Required Dental Care Program

This new program expands public coverage of oral healthcare to patients awaiting cardiac surgery, organ transplantation, or allogeneic hematopoietic stem cell transplantation.

Interim Federal Health Program (IFHP)

Co-payments for services and products requiring prior or special authorization.

Effective May 1, 2026, the Interim Federal Health Program (IFHP) will apply co-payments to all eligible supplemental health care services and products offered in Canada, including dental care for which prior or special authorization has been granted.

In Canada: Dental care for Quebec: updates include

  • Application of a 30% co-payment
  • Dental restorations: maximum of $1,000 per calendar year

Canadian Dental Benefit for Children Under 12

For families with an adjusted net income of less than $90,000 per year

What You Need to Know About the Canada Dental Care Plan
The Canada Dental Care Plan helps make dental care more affordable for eligible Canadian residents.
Oral health is an important part of your overall health and well-being.
Regular visits reduce the risk of developing health problems.

In 2022, nearly one in four Canadians reported avoiding visits to an oral health professional because of the cost.
All Canadians deserve access to affordable and accessible oral health care; it is essential.

Who is eligible for this program?
To be eligible for the Canada Dental Care Plan, you must meet the following four conditions:

  • Not have access to any type of private dental insurance.
  • Have an adjusted net family income of less than $90,000.
  • Be a Canadian resident for tax purposes.
  • Have filed an income tax return in the past year.

Start of Coverage
Once your application is accepted, you will receive a Sun Life Welcome Kit within 3 months, which will include:

  • Information about the Canadian Dental Care Plan (CDCP)
  • Your membership card
  • The start date of your insurance coverage

You can only receive covered dental care starting from the start date indicated in your Welcome Kit. Care received before this date will not be reimbursed.
You must present your membership card and valid photo identification upon arrival at the clinic.

Your Costs
At the dentist’s office, all patients, whether insured or not, pay the same rate for the same treatment.

Patients eligible for the Canada Dental Care Plan (CDCP) must pay the difference between their dentist’s regular fee and the amount reimbursed by the CDCP.

It is possible that the cost of dental care from your dentist may exceed the amount reimbursed by the CDCP.

You will therefore need to pay the balance to your dentist at the time of your appointment.

Be sure to know the amount you owe your dentist before receiving treatment.
